Background: The application of a patient safety culture at the hospital is something that absolutely must be applied in line with the work culture in the organization to be able to reduce the incidence of patient safety. The patient safety culture is the main foundation for patient safety. Objective: To analyze the effect of patient safety culture on patient safety incidents in A. Wahab Sjahranie Hospital. Method: an Analytical study with a cross-sectional design. 119 nurses in 14 rooms were sampled. The sample selection is done by proportional random sampling. The patient safety culture with 12 dimensions is an independent variable while the dependent variable is a safety incident. Data collection using a questionnaire refers to the AHRQ at the Hospital Survey on Patient Safety Culture in 2017. The patient safety incident questionnaire was prepared based on incident data that often occurs in the Hospital. Data were analyzed by multiple linear regression. Results: The mean nurses' perception of the overall culture of patient safety was 2.97 (SD ± 0.22). The lowest mean perception is on the dimension of non-punitive response to errors, which is 2.65 (SD ± 0.60). While the highest mean perception is on the dimensions of collaboration in units, namely 3.43 (SD ± 0.43). The type of incident that is very often is phlebitis which is equal to 8.4%. There was an influence of management support (p = 0.038, R = 5.633), nurses' perceptions (p = 0.015, R = 5.609) and cooperation between units (p = 0.006, R = 6.646) against incidents of patient safety Conclusion: The patient safety culture simultaneously has a significant effect on patient safety incidents in RSUD AW. Sjahranie Samarinda. Partially only the management support variables, nurses' perceptions, and cooperation between influential units. Management support, nurse perceptions, and collaboration between units need to be improved to prevent incidents in hospitals.
